We are grateful for the opportunity to minister to you through Delaware Bible Counseling and Discipleship Ministries.

Please fill out both forms completely online. If you have any trouble with the forms and would rather receive a printed version, please contact our office.

Once we receive your paperwork, we will call you to set up an appointment. We schedule appointments for Tuesdays at 6:30 pm and 7:45 pm. You will be assigned to the first available counselor, or if no counselor is available, your name will be placed on a waiting list. Either way, we will contact you.

If you have children, please arrange for someone to care for them at another location. We cannot provide childcare and the church nursery will not be available.
